Fees and Insurance

New patient evaluation, set three 60-minute visits - $600 each

45-minute follow-up visit - $400

25-minute follow-up visit - $300

Please note that our office does not accept insurance.  Payment is to be made at the time of service.  We accept credit, debit or HSA cards.  While our services are considered out of network, many insurance carriers have out-of-network benefits and may reimburse 20 to 90% of fees.  We provide a "superbill" that patients can submit to their insurance plan for reimbursement.

 

Patients have the right to a Good Faith Estimate of out-of-network healthcare costs.
